Adult Home Treatment Team

Adult Home Treatment Team

The Adult Home Treatment Team (HTT) service provides high level, hospital standard mental health care daily in your home as an alternative to going to or staying in hospital.

The service acts as a step between being admitted to hospital and community care. Providing intensive mental health assessment, treatment and management in your home means you can maintain daily routines, responsibilities and roles while still receiving the high-level care you need. The team will also put you in contact with other services such as community mental health teams or community-based organisations as required.

The team, consisting of a consultant psychiatrist, psychiatric registrar, junior medical officer, mental health nurses, an occupational therapist, social worker and pharmacist, will help you identify your needs and develop a treatment and support and discharge plan. Team members will treat you with respect and dignity at all times.

How HTT works

For up to 14 days, the team will provide the interventions and treatment to support your recovery in collaboration with you, your family or carers (with your consent and as appropriate).

Each day HTT clinicians will visit your home to monitor your treatment and progress. To improve your coping skills and support your recovery, the clinicians will also help you better understand your illness and medications. Medical review is provided by HTT doctors on a regular basis, at your home or in an appropriate location.

While you are in the care of the HTT, clinicians will maintain close links with your existing community care (Community Mental Health Team, GP and other community supports) to ensure appropriate care is available on discharge. 

Smoke-free policy

The Department of Health has a smoke-free policy. Smoking is not permitted in our buildings or grounds. For the protection of staff, we request you provide a smoke-free environment when receiving a visit from an HTT staff member in your place of residence.

Eligibility

People living in the Fiona Stanley Fremantle Hospitals Group (FSFHG) catchment area experiencing an acute episode of mental illness and requiring admission to hospital are eligible for this program. Refer to the SMHS mental health catchment map (PDF 262KB) to see suburbs that fall within the FSFHG catchment.

To participate, you must be willing to receive treatment at home rather than in hospital and agree to daily home visits.

You should also have access to a stable home living environment, and it must be safe for you and the team to visit you in your home. It is preferable you have supportive family, friends, or carers who are willing and able to be actively involved during this period.

Referral

Your GP can be referred to HTT via the Assessment and Treatment Team.

Your suitability for the service will be assessed and you will be admitted into the service as appropriate.

Appointments

HTT clinicians will make appointments with you at a time suitable for you and your support network. The service is available 7 days a week, 365 days a year. Appointments can be scheduled at home or location of your choice.

Team members try to arrive at the agreed time but sometimes unplanned events such as an emergency or traffic issues can cause delays. If this happens, we will call to notify you of the delay and re-arrange a convenient time.

The team are available via telephone from 8am–9pm daily to support you and your carers.

Mental health crisis services

Outside of HTT’s operating hours an emergency mental health service is available via the Mental Health Emergency Response Line (MHERL) by phoning 1300 555 788. This service is staffed by mental health professionals who provide expert advice to acute mental health issues. Other services include:

  • on a feedback form – your HTT clinician will provide you with a form
