Assessment and Treatment Team

The Assessment and Treatment Team (ATT) provides a triage mental health team which is the first point of access to mental health services offered at Fremantle Hospital.

The ATT provides specialist multidisciplinary community mental health assessment and treatment for people aged 18–65 years with mental health issues who live in the Fiona Stanley Fremantle Hospital Group (FSFHG) catchment area. Refer to the SMHS mental health catchment map (PDF 262KB) to see suburbs that fall within the FSFHG catchment.

Our outpatient multi-disciplinary team consists of:

  • consultant psychiatrist
  • senior mental health practitioner
  • psychiatric registrar
  • mental health nurses
  • occupational therapist
  • social workers.

Your general practitioner (GP) or other service provider can refer you to the Triage Team – see the referral section below for more information.

How the service works

When you first attend our service, our Triage Team will complete an initial assessment of your mental health.

If appropriate for our service you will be referred to the ATT which will work with you, your family and your GP to manage your care. Your personal aims and goals will be incorporated into this care.

Team members will help you to identify your needs and work with you to develop a recovery focused Treatment Support and Discharge Plan.

You may be provided a care coordinator for up to 10 weeks to support you in your recovery journey.

Most of your treatment time will be supported by your care coordinator. To ensure your safety and wellbeing at all times, however, you may also have contact with other clinicians who will work collaboratively to offer you the most appropriate service.

The ATT provide services in clinic and may do home visits if appropriate.

Our service is a smoke-free environment

Please note smoking is not permitted in our buildings or grounds as per the Department of Health’s smoke-free policy.

If you need continuing support

If you need support beyond the ATT’s 10-week care period, we will refer you to a service which may include:

  • our Community Treatment Team
  • a community managed organisation
  • your GP.

How to access our services

If you would like to access the ATT we recommend you ask your general practitioner (GP) for a referral. You may also self-present for this service.

Other hospitals and emergency departments, psychologists and community managed organisations can also refer to the ATT.

Refer a patient

After receiving your referral, the Triage Team will phone you to discuss your needs and determine which service is most suitable for you. You may be offered follow up with the ATT or if it appears our services are not appropriate to your needs you will be referred to your GP or signposted to the most appropriate service.

  • asking an ATT team member for a feedback form
  • completing an online feedback form
  •  phoning the Fremantle Hospital Patient and Family Liaison Service on 9431 2787.
