Community mental health services

Community mental health services

Multidisciplinary, recovery-focused care in the community is available to adults aged 18 to 65 years, and older adult consumers over 65, who:

  • have a mental illness
  • live in the Fiona Stanley Fremantle Hospitals Group (FSFHG) catchment area – refer to the SMHS mental health catchment map (PDF 262KB) to see suburbs that fall within the FSFHG catchment.
Community services

Adult Community Treatment Teams (CTT)

There are 5 community treatment teams:

  • Three of the teams are based in Melville, Cockburn and Fremantle.
  • The Early Intervention Psychosis (EIP) Service provides assessment, support and intensive care coordination to consumers who have suffered their first episode of psychosis. It also provides group work for consumers and carers.
  • The Assertive Community Treatment Team (ACTT) works with consumers with multiple complex needs, maintaining them in the community wherever possible.

Adult Home Treatment Team (HTT)

This service provides high level, intensive mental health care daily to consumers in their home as an alternative to going to or staying in hospital. The service acts as a step between being admitted to hospital and community care.

Fremantle Older Adult Community Mental Health Service

Fremantle Older Adult Community Mental Health Service (FOAMHS) is a specialist mental health service providing psychiatric assessment and treatment for older adults experiencing a mental health disorder. Read more about this service.

YouthReach South

This service aims to improve the function and quality of life for young people aged 13 to 24 years with mental health issues who live in the south metropolitan area and experience barriers to accessing mainstream services.

Other services

Hampton House (Karla Mia) provides community-based residential support, for up to 6 weeks, for adults experiencing significant functional deterioration as a result of their mental health. It operates as a step-up/step-down service, providing support for people who are at risk of becoming unwell and for people leaving hospital who need more intensive support before returning home.

Outpatient clinics provide scheduled appointments with the multidisciplinary team. Appointments are for review, assessment and care planning.

The Integrated Therapy Services (ITS) provides a range of programs for consumers and carers promoting the development of living, interpersonal, recreation and vocational skills.

How to access our services

The Assessment and Treatment Team (ATT) provides a triage mental health team which is the first point of access to the mental health services offered at Fremantle Hospital.

It operates from 8am–10pm, providing specialist multidisciplinary community mental health assessment and treatment services for individuals with mental health issues aged 18–65 years.

Patients may be referred to ATT by their general practitioner (GP) or other service providers.

Referrals from health professionals

Refer a patient

Self-referral

People who present to the mental health service without a referral will be triaged by ATT before being treated or referred to other services.

Support for carers

Carers have the main responsibility for care when service providers are not there. They are frequently responsible for providing care to consumers who are acutely unwell.

The mental health service:

  • ensures that carers and the essential role they play is identified at first contact, or as soon as possible thereafter
  • actively partners with carers to make sure that their family member or friend receives the best possible care.
