Your rehabilitation journey
You are now entering the next phase of your journey – rehabilitation at the State Rehabilitation Service at Fiona Stanley Hospital.
Rehabilitation is a time-limited, goal-orientated process which requires full cooperation and active participation between you, your support people such as family, friends or carers, and treating team.
Working together, you will set rehabilitation goals to:
- optimise your functional abilities
- promote independence
- preserve your self-esteem.
Your rehabilitation will focus on you undertaking tasks that are usual for you, such as dressing in your usual clothing during the day and not pyjamas. Part of your rehabilitation process will be improving your stamina to ensure you can capably undertake activities when you leave hospital.
Your progress towards reaching your goals will be reviewed regularly. If your progress is not occurring in the time frames given, your treating team will discuss an alternate plan with you and your support people. While complete recovery is not always possible, focussing on your goals will help you adjust to and accept any functional changes. This then enables you to gain an optimal level of function for the future.
