All Risk Continuity Midwifery Group Practice

All Risk Continuity Midwifery Group Practice

Our all-risk Midwifery Group Practice prioritises pregnancy and after birth care for women with high-risk pregnancy.

The same small group of midwives, in consultation with an obstetrician, will care for you:

  • throughout your pregnancy
  • during labour and birth
  • in the postnatal period.

There are two birthing options for women receiving care through this program. You may choose to birth in the:

  • Family Birth Centre (FBC) if you meet its eligibility criteria for birth (PDF 208KB)
  • main hospital birth suite for high-risk pregnancies.

If you have a low-risk pregnancy and no medical concerns, please apply for the Family Birthing Centre.

Benefits for you and your baby

Continuity of midwifery care has been shown to improve outcomes for families, including:

  • greater birth satisfaction for parents
  • safe birth outcomes for babies
  • excellent breastfeeding outcomes
  • reduced likelihood of birth interventions
  • shorter hospital stay
  • reduced likelihood of postnatal depression and access to support if necessary.

Most appointments throughout your pregnancy will be with your primary midwife.

In partnership with your midwife, you will discuss:

  • physical and emotional changes you may experience during pregnancy and birth
  • the importance of exercise and a healthy diet
  • changing family roles.

Who can use this service

You need to live within the Fiona Stanley Hospital catchment area postcodes:

  • 6148 – 6150
  • 6153 – 6160
  • 6162 – 6164
  • 6166.

There is no cost for this service for Medicare eligible patients.

How to apply

There are two-steps to apply for this model of care. Applications are required at both steps and need to be reviewed before you can be accepted.

Please note you will be contacted by one of our midwives once you are fully accepted into our service.

Step one – self-refer online

To self-refer, please either:

  • complete and submit the online self-referral form (external site)

or

  • download the application form (733KB) and either:
    • print the form, fill in your information and email us a scanned copy
    • save the form to your computer, fill in your information and email us the electronic form.

If you are tentatively accepted into our service, you will receive an email between weeks 8 and 12 of pregnancy (offers of a place with this service are sent monthly).

Your next step is to visit your general practitioner (GP) to arrange a formal medical referral.

Step two – your GP sends a referral

Ask your GP to send a completed GP Care referral form (PDF 127KB).

Please note you are not fully accepted into the service until a GP referral is received and assessed.

In the meantime, please continue your pregnancy care with your general practitioner (GP), including first trimester screening and prenatal blood tests.
