Cardiac patient education

Cardiac patient education

Fiona Stanley and Fremantle hospitals offer programs and education sessions to supports patients who have experienced or are at risk of cardiac (heart) conditions.

Cardiac rehabilitation program

Cardiac rehabilitation focuses on how exercise, education and lifestyle changes can help you:

  • make long-term lifestyle changes so you have a longer, healthier life
  • lower your risk of future cardiac issues.

Free group education sessions run through our Cardiac Rehabilitation Program will inform you how to better manage your condition or risk factors. Family members or careers are welcome to attend.

Eligibility

This program is recommended for patients with:

  • a history of:
    • heart attack
    • cardiac arrest
    • angina
    • coronary artery bypass surgery
    • angioplasty/coronary artery stents
    • heart failure/cardiomyopathy
    • arrhythmia, including atrial fibrillation or atrial flutter
  • risk factors for heart disease, including:
  • diabetes
    • high blood pressure
    • high cholesterol
    • family history of heart disease
    • smoking
    • obesity.

Session information

The program consists of group education sessions held weekly on a Wednesday morning. You can participate online or by attending a face-to-face session at the hospital.

The sessions are presented by cardiac rehabilitation team members including:

  • cardiac rehabilitation nurse
  • exercise specialist
  • physiotherapist
  • dietician
  • social worker
  • occupational therapist
  • pharmacists.

Please register at for your session at 9.30am – presentations start at 9.45am and finish at 12.15pm (2hrs 30 minutes running time).

The sessions are structured as follows:

Session A

9.30am – Introduction and registration

9.45am – Understanding how your heart works, angina vs heart attack, and cardiac risk factors

10.45am – How your medications work and possible side effects

11.30am – Understanding emotional changes

12:15 pm – Morning session finishes

 

Session B

9.30am – Introduction and registration

9.45am – Exercise safely and regularly within your limits

10.30am – Learning to manage stress

11.15am – Improve your diet with heart healthy food choices

12.15am – Group education session complete

Videoconference

Please follow the steps below so you are ready to join the session at the nominated start time.

  1. Install Avaya workplace on your device (this can be found in your device app store). If you do not have the Avaya app, copy and paste the following in your search bar: https://telehealth.health.wa.gov.au/portal/tenants/wahealth
  2. To join the session, tap on the Avaya App and type your full name and the meeting ID 6840.

Implantable cardioverter defibrillator education

Monthly group education sessions are available to patients of Fiona Stanley and Fremantle hospitals who recently had an implantable cardioverter defibrillator (ICD) device implanted.

An ICD works by sending an electrical shock to the heart when it detects a life-threatening heartbeat, changing it from an abnormal to normal rhythm.

Participating in a session will help you adjust to life with your ICD and provide you with a support network.

Topics covered include:

  • the anatomy of your heart
  • how your heart works
  • what is an abnormal rhythm
  • what an ICD looks like and how it functions
  • the difference between a pacemaker and ICD
  • home monitoring
  • adjusting to life with an ICD.

How to participate

You can self-refer to participate in these sessions. Please register by phoning 6152 5480 or 6152 1573.

Sessions are held on the first Monday of every month, and you can attend a face-to-face session at the hospital or participate online.

Face-to-face
