Ophthalmology referrals

Ophthalmology referrals

Fremantle Hospital Ophthalmology Department is the referral centre for all ophthalmic conditions in the South Metropolitan Health Service (SMHS) catchment.

Referrals (including from optometrists)

For referral requirements, please refer to the Ophthalmology referral criteria – Adult (external site),

Routine referrals from medical practitioners (including general practitioners and ophthalmologists) are to be directed to the Central Referral Service (external site).

It is preferred that routine referrals from optometrists are emailed to Fremantle Hospital Central Referrals with PDF attachment in one document only. Otherwise, please fax to Fremantle Hospital Central Receipting on 9431 2009.

Cataract referrals

For specific requirements for cataract referrals, please refer to the Cataracts and assessment for cataract surgery – Adult (external site).

Urgent referrals

For all urgent referrals, during clinic opening hours, referring health practitioners are asked to contact the on-call ophthalmology registrar via Fremantle Hospital switchboard on 9431 3333.

Out of hours, weekends and public holidays, referrers are to contact the on-call ophthalmology registrar via Royal Perth Hospital switchboard on 9224 2244.

Please note the department does not accept walk in referrals.

8:00am to 5:00pm, Monday to Thursday

8:00am –1:00pm, Fridays
