Parking infringement notices
Parking infringement notices (parking fines) may be issued by an authorised officer to any vehicle parked at any metropolitan hospital in contravention of the Health Services (Conduct and Traffic) Regulations 2016 (external PDF 94KB).

Infringement notices are part of a legal process and should not be ignored.
The Fremantle Hospital Parking Department administers the infringement process for the following South Metropolitan Health Service hospital sites:
- Fiona Stanley Hospital
- Fremantle Hospital
- Rockingham Hospital.
If you receive an infringement notice for another metropolitan hospital site, please refer to the instructions on your infringement or visit the hospital’s web site for more information.
If you receive an infringement notice
The following options are available to you on or before the due date.
A modified penalty is usually lower than the penalty that apply if the same matter could be dealt with by a court.
You can pay the modified penalty on or before the due date:
- on credit card by phoning 9431 3733 between 8am and 4pm, Monday to Friday
- via BPay (biller code details can be found on the infringement notice).
Paying the modified penalty is not regarded as an admission for the purposes of any civil or criminal court case.
If you need more time to pay, please phone the Fremantle Hospital Parking Department on 9431 3733 or email the Parking Department.
The responsible person of the vehicle is presumed to be the driver or person in charge of the vehicle unless the following information is provided via the statutory declaration form below:
- the name and address of the driver or person in charge of the vehicle at the time of the alleged offence
or
- information showing that at the time of the alleged offence the vehicle had been stolen, unlawfully taken, or was being unlawfully used.
Please:
- Complete this in the presence of an authorised witness.
- Post the completed form to Fremantle Hospital Parking Department, P Block, 2 Alma Street, FREMANTLE WA 6160

If you consider you have good reason to have this notice withdrawn, you may dispute the offence informally in writing.
To avoid enforcement action and additional fees, appeals need to be submitted on or before the notice due date. A printable infringement appeal form (PDF 48KB) Is available.
Please complete and submit your appeal form by:
- posting it to Fremantle Hospital Parking Department, P Block, 2 Alma Street, FREMANTLE WA 6160
- emailing the Parking Department.
You may wish to attend court in relation to the offence you are alleged to have committed. This should be considered your final option as costs may be incurred if the court find against you
To elect to go to court:
- complete the ‘Election to go to Court’ section on your infringement notice
- post it to Fremantle Hospital Parking Department, P Block, 2 Alma Street, FREMANTLE 6160.
Outstanding infringement fees
If you choose not to follow one of the options available and the infringement notice remains unpaid your you may be prosecuted or enforcement action may be taken under the Fines, Penalties and Infringement Notices Enforcement Act 1994 (external site).
If you are prosecuted in a court and convicted of this offence, the penalty can be imposed and court costs will be added.
If enforcement proceedings are taken against you and you fail to pay
- your driver’s licence and/or vehicle licence may be suspended or cancelled
- you may be disqualified from holding or obtaining a drivers or vehicle licence
- your personal details may be published on a website
- your vehicle may be immobilised or have its number plates removed
- your earnings or bank accounts may be withheld
- your property may be seized and sold.
