Radiology referrals (Fremantle Hospital)

Radiology referrals (Fremantle Hospital)

Radiology at Fremantle Hospital provides services to inpatients and non-admitted patients via booked appointments and inpatient referral.

These services include:

  • general X-ray
  • ultrasound, including general, MSK, obstetric and vascular
  • computerised tomography (CT) scans
  • bone mineral densitometry (BMD)
  • magnetic resonance imaging (MRI)
  • radiographic fluoroscopy.

Routine bookings

Routine bookings are made on receipt of a completed and signed radiology request form or an electronic form submitted via CPOE. Please phone 9431 2301 to request an x-ray request form.

Requesting doctors must include on the form their:

  • full name
  • pager or mobile number
  • provider number
  • consultant.

The form must be signed. Incomplete or illegible forms will be returned to the requesting doctor for amendment.

Please fax your request form to 9431 2663 and give the original form to your patient.

Radiology request forms received after 4.30pm are processed the following weekday morning.

Pregnancy status of female patients

The pregnancy status of female patients between the ages of 12 and 50 should be checked, especially if irradiating the pelvic/abdominal area.

Requesting doctors are responsible for advising the department of any risks to their patient.

Images and reports

Reports will be distributed to the referring practitioner via HealthLink or fax.

Operating hours

8.30am–4.30pm, Monday to Friday.

An on-call Medical Imaging Technologist is available outside these hours for urgent CT and general x-ray examinations only. Please contact through switchboard on 9431 3333.
